Covid-19 Vaccine Clinic

On Friday, August 20th, the Union Community School District is hosting a free Covid-19 vaccine clinic. The vaccine is not a requirement of the district. The UCSD wanted to provide another opportunity to receive the vaccine if you are still interested. The clinic is open to anyone in the community age 12 and up.

This Pfizer vaccine clinic will take place on Friday, August 20th at the Union High School from 8:00 am – 10:00 am. Below is a consent form that needs to be filled out by a parent or guardian for anyone under age 18. If the form is filled out and signed, parents do not need to be present for their child to receive the vaccine.
